SummitWest Care is seeking a Part-Time or PRN Speech-Language Pathologist (SLP) to provide one-on-one therapy in the home health setting across Mesa County. Why SLPs Choose Home Health with SummitWest Care Flexible scheduling – tell us when you're available No productivity quotas – focus on outcomes, not numbers One-on-one sessions – no distractions, no double-booking Clinical autonomy – practice at the top of your license Support when you need it – collaborative interdisciplinary team What You'll Do Evaluate and treat patients with speech, language, cognitive, voice, and swallowing disorders Develop and implement individualized, functional treatment plans Provide dysphagia management and cognitive-communication therapy Educate patients and caregivers on strategies to improve communication, safety, and independence Collaborate with an interdisciplinary team (RNs, PTs, OTs, social workers, caregivers) Complete documentation in Homecare Homebase (HCHB) – training provided Travel locally within Grand Junction, Fruita, Clifton, and Palisade What You Bring Active Colorado Speech-Language Pathologist (SLP) license CCC-SLP preferred (CFs may be considered depending on experience optional if you want to open pipeline) Strong clinical judgment in cognitive and swallowing disorders Ability to work independently in a home-based setting Clear, compassionate communication style Reliable vehicle, valid driver's license, and auto insurance Home health or medical experience preferred (we will train the right candidate) Pay & Perks ?
