Join us and start doingyour life's best work.SM As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education.CHWs work in a team-based structure and spend the majority of their time in the community engaging directly with members.Interesting in learning to work with medically complex patients who may be experiencing significant addiction and/or behavioral health conditions is important for this role. This position is full-time (40 hours/week) Monday Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00am to 5:00pm. It may be necessary, given the business need, to work occasional overtime. This position is a field-based position with a home-based office. You will work from home when not in the field. Essential Duties and Responsibilities Responsible for: intake/screening/referral, assessment/reassessment, development of support plans, on-going case management, monitoring of the Members health and welfare, documentation of contacts and case management activities in the Department-prescribed IMS, resource development, and case closure. Respect confidentiality and maintain confidences as described in the UHG Employee Handbook and acknowledged through signature by all employees. Maintain confidentiality of patient information. The ability to maintain confidentiality is a critical and essential component of this position. Contact the Members at least once within each quarterly period, or more frequently if warranted by the Members condition /or as determined by the rules of the LTSS Program in which the Members is enrolled. Conduct face-to-face contact with the Members at least every six (6) months, or more frequently if warranted by the Members condition or the rules of the LTSS Program in which the Member is enrolled, and shall update the current approved assessment tool and Support Plan in the appropriate documentation system to reflect any changes in condition or services. Complete a new assessment during a face-to-face reassessment annually, or more frequently if warranted by the Members condition or if required by the rules of the LTSS Program in which the Member is enrolled. Complete additional assessments as needed/required. Monitor the delivery of services and supports identified within the Support Plan and the Prior Authorization Request (PAR). This includes monitoring:

The quality of services and supports provided The health and safety of the individual; and The utilization of services with respect to the Support Plan and the PAR. The following criteria may be used to determine the Members level of need for case management services: Availability of family, volunteer, or other support Overall level of functioning Mental status or cognitive functioning Duration of disabilities Whether the individual is in a crisis or acute situation The individuals perception of need and dependency on services The individuals move to a new housing alternative Whether the individual was discharged from a hospital or Nursing Facility. Attend Department provided training for SEP agencies as well as agency provided in-service and staff development training. Responsible to conduct work that is in accordance with the SEP Contract Agreements requirements. This includes but is not limited to, providing work that is accurate, timely, and complete. May take on special assignments and projects on an as needed basis. Assist with system assessment and development as needed. This position requires travel. Requires independent, reliable, flexible, and on-demand, transportation at the incumbents expense for travel between various locations and timely arrival and departure from various locations. If the employee chooses to satisfy this requirement by driving a vehicle, the employee must meet the requirements for Colorado licensure and company requirements for liability insurance coverage. Additional Duties and Responsibilities Complete all duties in accordance with company safety policies and practices. Required Qualifications: Bachelors degree (or higher) in one of the human behavioral science fields such as human services, nursing, social work, psychology, public health, or the combination of education and experience that would enable performance of the full scope of the position OR experience as a caseworker or case manager with LTSS population, in a private or public social service agency Required to travel up to 50% within the State of Colorado on a daily basis while being exposed to changing weather conditions. Travel will include various locations where timely arrival and departure is important. Required to work a flexible schedule including before and after core business hours and occasional evenings and weekends as business needs dictate Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role Employees arerequiredto screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained. Soft skills: Ability to execute regular, reliable, and predictable performance of the essential functions of this job through a consistent ability to be physically and mentally present at the assigned work location, at the prescribed time, with the prescribed tools and with the availability necessary to successfully complete the essential duties described herein. Ability to handle and manage stress associated with the performance of job duties. Ability to appropriately interact with, and get along with others including supervisors, coworkers, customers and other stakeholders. Ability to work collaboratively with others as set forth in the job description. Ability to accurately and timely follow directions of supervisors. Ability to perform the essential functions of the job at the assigned primary work location for this job, which is in the employees home. Ability to effectively speak, understand and be understood, and communicate in English. Ability to hear adequately on the phone, in person and in group settings. Ability to provide independent, reliable, flexible, and on-demand, transportation at the employees expense. If the employee chooses to satisfy this requirement by driving a vehicle, requires ability to meet the requirements for Colorado licensure and company requirements for liability insurance coverage. Ability to work in front of a computer screen and keyboard, sitting and/or standing up to 8-10 hours per day (possibly longer if required). Ability to work in home settings that include varying physical, social, and cultural environments. Ability to work in environments that may have exposure to communicable disease while performing patient care activities. Ability to see adequately to visually evaluate and assess the members or potential members physical condition and living environment. Ability to see computer screen and work papers. Ability to access, research and interpret information from a variety of media (e.g., computer screen, projected images, printed material) and individuals.) Ability to work in normal office environment conditions and with various office equipment (e.g., computer, keyboard, mouse, calculator, copier, printer, fax, scanner, telephone). Ability to move to access various offices and a wide variety of meeting settings. Ability to perform a variety of tasks that involve standing, walking, sitting, stooping, kneeling, bending and twisting, occasionally climbing stairs or using an elevator, possibly reaching chest high and overhead for materials.
