Join a traveling Physical Therapist role in Grand Junction, CO, Home Health where you will provide skilled physical therapy services to patients in their homes and experience life in a Western Colorado setting. Apply now and a recruiter will contact you with more details. Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Colorado Physical Therapist license required to start the assignment. BLS certification from AHA or ARC may be required. Home health experience preferred. Experience with HomeCare HomeBase (HCHB) preferred. Ability to complete comprehensive evaluations, develop measurable goals, and document treatment according to company, funding source, and regulatory requirements. Experience supervising PTAs and PT aides in accordance with state practice act preferred. Willingness to travel within assigned counties including Mesa County and Garfield County as part of the caseload. Productivity expectation: minimum of 30 visits per week. Assignment Details Facility type: Home Health. Shift: Days. Shift hours: 0800-1700, Monday through Friday. Counties covered: Mesa County and Garfield County. Assignment type: Travel contract to hire.
