Why This Role Stands Out Consistent Monday–Friday schedule Manageable caseloads (no unrealistic productivity quotas) One-on-one patient care in the home setting Supportive leadership and clinical collaboration What You'll Do Provide skilled nursing care in a home health setting Complete comprehensive patient assessments and develop individualized plans of care Monitor patient progress and adjust care plans as needed Educate patients and families on medications, treatments, and self-care Coordinate care with an interdisciplinary team (LPNs, therapists, caregivers) Complete documentation in Homecare Homebase (HCHB) – training provided Participate in a rotating on-call schedule Travel locally within Grand Junction, Fruita, Clifton, and Palisade What You Bring Active Colorado Registered Nurse (RN) license Strong clinical assessment and critical thinking skills Ability to work independently while managing a structured caseload Clear, professional communication style Comfort with documentation and basic technology Reliable vehicle, valid driver's license, and auto insurance Home health experience preferred (we will train the right candidate) Pay & Benefits ?
