What You'll Do Provide skilled nursing care in a home health setting under physician direction Perform in-home visits, including treatments, monitoring, and follow-up care Administer medications and treatments within LPN scope of practice Observe and report changes in patient condition to the RN and care team Educate patients and families on medications, disease management, and safety Document visits in Homecare Homebase (HCHB) – training provided Collaborate with an interdisciplinary team (RNs, therapists, caregivers) Travel locally within Mesa County (Grand Junction, Fruita, Clifton, Palisade) What You Bring Active Colorado LPN license (unrestricted), IV Certification a plus! Ability to work independently and manage your schedule in the field Strong communication and patient-centered care approach Comfort with documentation and basic technology Valid driver's license, reliable vehicle, and auto insurance Home health or community-based care experience preferred (we will train the right candidate) Why LPNs Choose SummitWest Care ?
