What we offer Competitive salary $50,000.00 - $ 65,000.00 Benefits include medical, dental, vision, generous Paid Time Off program, holidays, 401k and more A growing company with opportunities for advancement Company sponsored training, tuition reimbursement, and other learning opportunities Job Description The Director of Social Services is responsible for planning, developing, organizing, implementing and directing the SNF Health & Wellness in accordance with current existing federal, state and local regulations, as well as Stellar Senior Living established policies and procedures, to ensure medically related social services are provided to all residents in the facility. Responsibilities Plan, organize, develop the overall operation of the Social Services department to ensure all medically related emotional and social needs of patients are met Ensures the quality of social services rendered by the facility at all times Participate in the admission, discharge, MDS coordination, etc for social services Refers patients and families to appropriate community resources as necessary Qualifications Bachelor’s Degree in Social Work or equivalent At least two years of experience in Long Term Care preferred. Demonstrated knowledge or rules, regulations and guidelines that govern nursing care facilities.
