based in Utah, is seeking a dedicated and compassionate Social Work
Hospital Care Manager to join our team. This pivotal role involves
working across our hospitals and clinics to provide holistic
patient care management. The ideal candidate will have a robust
background in social services within healthcare settings, equipped
to devise care plans that cater to the unique needs of each
patient, ensuring their seamless transition through different care
stages and supporting their well-being. This position demands a
high level of collaboration with healthcare professionals to
optimize patient outcomes and uphold the standards of excellence
synonymous with Intermountain Healthcare. Assess patient needs and
develop individualized care plans. Collaborate with healthcare
providers to coordinate patient care. Advocate for patients' access
to community resources. Monitor and evaluate patient progress and
adjust care plans as needed. Support patient families with
education and resources related to healthcare processes. Maintain
meticulous records of patient interactions, treatments, and
progress. Strong interpersonal and communication skills Ability to
work collaboratively in a multidisciplinary team Knowledge of
healthcare systems and community resources Critical thinking and
problem-solving capabilities Capacity for empathy and compassion in
high-stress environments Medical Insurance Dental Insurance Vision

(LCSW) or equivalent state licensure. Certification in Case
Management (CCM) preferred.
