The Physical Therapist will deliver skilled physical therapy services in patients' homes in accordance with physician orders, payer requirements, agency policies, and applicable state and federal regulations. This role supports patient mobility, safety, and functional independence through individualized, evidence-based care. Clinical Responsibilities include performing assigned physical therapy visits, including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits. The Physical Therapist will develop and implement individualized physical therapy plans of care based on patient needs and physician orders. They will provide skilled interventions such as gait training, balance training, therapeutic exercise, mobility training, and fall prevention. Educating patients and caregivers on treatment plans, home exercise programs, and safety strategies is also part of this role. Communication of patient progress, concerns, or changes in condition to the interdisciplinary care team is essential. Collaboration in the clinical oversight of Physical Therapist Assistants (PTAs) is expected. Documentation Responsibilities involve completing all required documentation accurately and timely in Homecare Homebase (HCHB). Documentation must comply with Medicare Conditions of Participation, payer requirements, and agency standards. Maintaining compliance with privacy and security requirements is crucial. Care Coordination includes collaborating with clinical staff and participating in care coordination related to patient needs, visit completion, and regulatory requirements. Providing clinical oversight of PTAs may be needed when applicable, assigned, and permitted under agency policy. This position offers a patient-centered, in-home care environment with one-on-one visits and supportive clinical and administrative coordination.
