The Position In the role of Physical Therapist (PT), you will
assess the physical capabilities of residents and create tailored
treatment plans to assist them in regaining or maintaining
mobility, functionality, and independence. You will deliver
hands-on therapy, educate patients and their families, and
collaborate with interdisciplinary teams to ensure the best
possible outcomes. Main Responsibilities Perform thorough
assessments of residents’ physical abilities and mobility using
standardized assessment tools. Create and execute evidence-based
treatment plans aimed at enhancing strength, balance, coordination,
and range of motion. Deliver therapeutic interventions, including
exercise regimens, gait training, and the use of assistive devices.
Provide education to residents, families, and staff on safe
mobility practices and home exercise routines. Accurately document
evaluations, progress notes, and discharge summaries in a timely
manner, adhering to regulatory and organizational standards. Work
collaboratively with occupational therapists, speech-language
pathologists, nurses, and other team members to provide coordinated
care. Ensure compliance with all relevant state and federal
regulations. Qualifications Current licensure as a Physical
Therapist (PT) in the relevant state. Graduation from an accredited
Physical Therapy program. Experience in long-term care, skilled
nursing, or rehabilitation is preferred. Excellent communication,
documentation, and organizational abilities. A compassionate
