Now Hiring: Licensed Practical Nurse (LPN) Location: Mantey Heights Rehabilitation & Care Center – Grand Junction, CO Schedule: Full-time, Part-Time day/nights & PRN | 12-Hour Shifts (6:00 – 6:30 w/30 min lunch) & Evening Shifts (6:00 PM – 10:00 PM) Mantey Heights Rehabilitation & Care Center is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our nursing team. Position Overview As an LPN, you will provide skilled nursing care under the direction of a Registered Nurse (RN) or physician. You will play a vital role in monitoring resident health, administering medications, and providing compassionate support to residents and their families. Key Responsibilities Administer medications and treatments according to physician orders and facility policy. Monitor residents’ health, including vital signs and changes in condition, and promptly report concerns to the RN or physician. Assist with resident admissions, transfers, and discharges. Ensure accurate and timely documentation in medical records. Support CNAs and other team members by providing guidance and assistance. Promote and protect resident rights, dignity, and safety at all times. Collaborate with interdisciplinary team members to provide holistic resident care. Qualifications Current LPN license in the State of Colorado (required). Ability to work 12-hour day and night shifts (6:00 – 6:30 w/30 min lunch) and/or short evening shifts (6:00 PM – 10:00 PM) . Strong clinical and assessment skills. Excellent communication and teamwork abilities. A positive, compassionate attitude toward residents and coworkers. Previous experience in long-term care or skilled nursing preferred, but not required. What We Offer Competitive wages: $26.00 – $39.00 per hour DOE . $10,000.00 Sign on Bonus (full-time position only) Comprehensive benefits package for eligible employees (medical, dental, vision).
