Job Description:
FUNCTION: Provides nursing care, nursing services and health
education to nursing home residents. QUALIFICATIONS: The
requirements listed below are representative of the knowledge,
skill and/or ability required. Minimum Qualifications: --- Licensed
L.P.N., preferably with experience in long-term care or geriatric
nursing. --- Current and unencumbered state license to practice as
a nurse. --- Current CPR certification and additional certification
in nursing specialty desired. --- Demonstrates ability to read,
write, and speak English sufficient to fulfill job requirements.
--- Demonstrates the ability to make independent decisions, solve
practical problems that may or may not include a variety of
concrete variables. This may require the ability to interpret
instructions given in written, oral, diagram, or scheduled form.
--- Demonstrates the ability to listen actively and evaluate social
situations in an objective, consistent and fair manner. Functions
independently with integrity, flexibility and works well with
residents, personnel and support agencies. ESSENTIAL FUNCTIONS AND
RESPONSIBILITIES: To perform this job successfully, an individual
must be able to perform each key function satisfactorily. 1.
Institutes appropriate nursing intervention which might be required
to stabilize a resident's condition and/or prevent complication. 2.
Makes daily resident rounds to determine physical and emotional
status and to implement any required nursing interventions. 3.
Responds to resident and family concerns and ensures that each
concern is documented and a resolution is initiated or communicated
to appropriate staff. 4. Coordinates admission, discharge and
transfer of residents. 5. Obtains report from nurse he or she is
relieving, provides report to nurse coming on duty and keeps the
charge nurse or others (as appropriate) informed about the status
of residents and related matters. 6. Prepares, administers and
documents medications following accepted practices. 7. Consults
with licensed or registered nurse, physician or licensed
independent practitioner. 8. Clarifies any order or treatment
regimen believed to be inaccurate, ineffective or contraindicated
by consulting with the appropriate licensed practitioner. 9.
Notifies the supervising nurse when medication or treatment issues
arise. Assists in discussion making process. 10.Knows the rationale
for the effect of medications and treatments and correctly
administers same. 11.Accurately reports and documents the
resident's symptoms, responses and status. 12.Monitors residents
who self-administer medication and reports deviation from the plan
of care to the licensed or registered nurse. 13.Administers
medications within 60 minutes of schedule time. 14.Demonstrates
knowledge or and complies with proper medication administration
policies and procedures. 15.Lists all current medications and
dosage schedules on the resident's medication or administration
record. 16.Reports a drug administration error or drug reaction to
the charge nurse and completes report when appropriate. 17.Removes
discontinued, outdated or deteriorated drugs from inventory.
18.Reviews medication and treatment records for completeness,
accuracy in transcription of physician orders and adherence to stop
order policies. 19.Orders and documents receipt of medications.
20.Supervises the serving and documentation of prescribed diets and
fluid intake. 21.Counts all narcotics, signs for count and
exchanges keys to medication carts and medication room. 22.Checks
emergency equipment (oxygen and suction) and emergency medication
kit (if application) 23.Performs other duties as assigned by the
DON. 24.All equipment is operated in a safe manner and the only
equipment utilized is that which previous training of use has
occurred. 25.Defective equipment is reported to the Manager. 26.The
nurses station is maintained a. Supplies are maintained at par
levels and are arranged in an organized manner. b. Nurses station
is neat and orderly. 27.--Assure cleaning and maintenance occurs
according to schedule. a. IV or tube feeding pump poles, tube
feeding infusion pumps, and suction machines are wiped down weekly.
b. Utility room is checked routinely for supplies that need to be
returned to the appropriate department, meal trays requiring return
to dietary, and linen that requires removal to the laundry. c.
Safety hazards are removed from unit floors (liquid spills, food,
paper, equipment cords, etc.). 28. Assure the resident's
environment is safe and clean. 29. Individual patient/resident
supplies are restocked as indicated. 30. Procedures are performed
according to established method in the procedure manual. 31. Body
substance precautions and other appropriate infection control
practices are utilized with all activities. 32. Safety guidelines
established by the facility (i.e. proper restraint and side-rail
use) are followed. 33. All accidents or incidents observed are
reported on the shift that they occur. 34. Intake and output is
accurately recorded on the appropriate form. 35. Patient/resident
weights are recorded on the appropriate form. 36. All changes in a
residents condition are reported as soon as possible to the
appropriate parties. 37. Staff meetings are attended, if on duty.
Minutes read and initialed if not on duty. 38. Resident/unit
problems are reported to the Supervisor/Manager. 39. Decisions are
made that reflect both knowledge and good judgment. 40. Awareness
of own limitation is evident and assistance is sought when
necessary 41. Dress code is adhered to. 42. Committee meetings (if
assigned) are attended. Reports related to the committee are given
during staff meetings. 43.--Responsibility is taken for own
professional growth. Attend and participate in all scheduled
training, educational and orientation classes, programs and
activities including annual mandatory classes. 44.--Follow work
schedules and complete assigned tasks according to the established
policies and procedures of the facility. 45. Complies with
Attendance/Tardiness policy. 46. Confidentiality of resident is
respected at all times (i.e., when answering telephone and/or
speaking to co-workers or use of fax or electronic messaging.) 47.
Cooperates with inter-departmental personnel to ensure that the
needs of the residents are met and maintained. 48. Demonstrates a
positive working relationship with residents, visitors and facility
staff is demonstrated. 49. Authority is acknowledged, and response
to the direction of supervisors is appropriate. 50. Time is spent
with residents rather than other personnel. 51. Co-workers are
readily assisted as needed. 52. Participate as requested by the
Medical Director or Director of Nursing Services in the planning
and conducting of in-service training classes 53. Participate in
educational programs for residents and their family members when
resident is being discharged to home. 54. Attend and participate in
annual in-service programs for hazard communication, TB management,
bloodborne pathogens standard, and other related programs. 55.
Attend and participate in advance directive in-service programs for
the staff and community. 56. Assist in monitoring the inventory of
medications, medical supplies, and equipment to ensure an adequate
supply of skin care products are on hand to meet the needs of
residents. 57. Be familiar with and use as appropriate all items of
personal protective equipment offered by the facility, including,
but not limited to, masks, gowns, gloves, and back brace to be worn
when lifting. 58. Wear and/or use safety equipment and supplies
(e.g., back brace, mechanical lifts, etc.) when moving or lifting
residents. Resident Rights: 59. Maintain the confidentiality of all
resident care information. 60. Monitor all care and activities of
residents to ensure that residents are treated fairly, and with
kindness, dignity and respect. 61.Report any complaints or
grievances made by residents regarding nursing or medical care to
the Director of Nursing Services. Maintain a written record of any
resident's complaints or grievances communicated to you with a
notation of actions taken. 62. Report and investigate all
allegations of resident abuse and/or misappropriation or resident
property. 63. Ensure that nursing staff personnel honor the
resident's refusal of treatment requests and that such requests are
in accordance with the facility's policies governing advance
directives. 64.Complies with community policies and procedures for
workplace safety including infection control procedures, emergency
response and safety protocol. 65.Complies with all company policies
and procedures related to resident rights; abuse prevention and
reporting; and resident confidentiality (HIPPA). 66.Demonstrates
knowledge of Employee Handbook and complies with the policies in
the Handbook. 67.Performs other related duties as may be assigned.

Seeing: Must be able to read reports and use computer. Hearing:
Must be able to hear well enough to communicate with co-workers,
residents. Standing/Walking/Mobility: Must be able to reach above
his/her head and perform repetitive movements on feet over long
distances. Climbing/Stooping/Kneeling: Must be able to bend to the
floor. Lifting/Pulling/Pushing: Must be able to transfer up to 50
pounds. Fingering/Grasping/Feeling: Must have finger dexterity to
